About the RoleMass General Brigham is seeking a skilled Registered Nurse to fill the position of Case Manager. This role is responsible for coordinating patient care, ensuring timely and effective transitions, and promoting optimal health outcomes.Key Responsibilities

• Develop and implement individualized care plans, taking into account patient needs, preferences, and clinical requirements.
• Collaborate with interdisciplinary teams to ensure seamless care coordination, including physicians, nurses, and other healthcare professionals.
• Conduct thorough assessments of patient needs, identifying opportunities for improvement and implementing strategies to address them.
• Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care.
• Monitor and evaluate patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
• Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies.Requirements
  • Graduate of an approved school of nursing with current registration in Massachusetts.
  • 1+ years of acute care experience, with a focus on case management or a related field.
