Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate complex care management activities for members.
Key Responsibilities
Evaluate member needs and barriers to care, recommending plans for
optimal outcomes
Develop and coordinate ongoing care plans while collaborating with
providers and community resources
Monitor member status and progress towards care plan goals,
providing necessary updates and interventions
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
