The Nurse Care Manager serves as care manager to members across the continuum and collaborates with care managers from vended programs.Job DescriptionWHAT YOU WILL BE DOING

  • Assesses, plans, implements, documents, coordinates, monitors, and evaluates the plan of care by collaborating with the members, family members/DPR, physician, the health care team, providers, and employer groups, as appropriate.
  • Researches, evaluates, and recommends resources to meet medical and non-medical needs of members.
  • Collaborates, refers, and communicates across all programs to ensure appropriate coordination of services. Identifies problems and possible solutions in the coordination of care to determine most appropriate setting.
  • Applies motivational interviewing techniques to engage members in achieving their health goals Functions as a liaison to promote effective working relationships with providers and as a resource/ consultant for other nurse care managers and departments within Point32Health.
  • Reports clinical occurrences to appropriate designee.
  • Performs concurrent review using Point32Health criteria and guidelines to determine when services are no longer medically necessary and to transition to appropriate level of care.
  • Perform out-of-the-box thinking, collaborate with others, and make a difference every day!
  • Other duties and projects as assigned.RequirementsWHAT YOU NEED
    • Registered nurse, BSN required or appropriate experience and/or education.
    • Minimum 5 years of professional clinical experience in a clinical setting; Active licensure in required states with Certification in a specialty area.
    • Experience with care management, discharge planning, and electronic medical record systems.
    • Patient teaching/education/home care background; knowledge in motivational interviewing techniques.
    • Ability to sway others in acceptance of an idea, plan, or activity; able to assess both physical and psychosocial problems.
    • Car and Valid Driver's License, excellent telephonic problem solving, organizational, communication, and interpersonal skills.
