Job Description:
Under the direction of the Supervisor of Release of Health
Information Services Department (TBD), this position is responsible
for processing written and electronic requests for medical
information to patients or authorized parties in accordance with
HIPAA, General Laws of the Commonwealth of Massachusetts (MGL),
Department of Public Health (DPH), hospital and department policies
and procedures. The Release of Information Medical Abstractor will
ensure timely responses to said requests and maintains the
confidentiality and integrity of all information. This position
provides direct customer point of service and supports the overall
release of information process for internal and external customers.
Principal Duties and Responsibilities Accurately abstract and or
prepare records by screening and analyzing health records for any
specifically protected or privileged information that is not
covered under a general authorization. When required, obtains
proper authorization for records containing treatment of alcohol
and drug abuse,42 CFR Part 2, sexual transmitted disease, genetic
screening results, licensed social worker, AIDS/HIV, mental health,
domestic violence counseling, and sexual assault counseling, as
defined by state and federal law, specifically stating the type of
information that is being released. Ability to redact information
manually and/or electronically when necessary. Understands and
enforces hospital confidentiality policies and procedures, ROI
department policies and procedures including Massachusetts General
Laws (MGL), Federal Public Health Laws, HITECH and HIPAA regulatory
guidelines. Acquires and maintains a working knowledge of medical
terminology and applicable department processes and applications.
Responsibilities include working knowledge of the Health
Information Services (HIS) Terminal Digit filing system. Able to
effectively utilize PeC, CAS, LMR, MUSE, secure delivery server,
fax server, Adobe (searching, redacting and securing security
settings), scanning documents, merging files, archiving system,
etc. Maintain a working knowledge of faxing workflow application
and utilizes functionality to index faxed/scanned images of
authorizations and to access assigned authorizations from work
queues in order to fulfill requests. Acquires and maintains a
thorough working knowledge of Biscom, Faxcom and Biscom Secure
Delivery Server to transmit medical record information securely in
an electronic format. Utilize PeC work drivers and electronic work
queues daily, to sort requests according to date needed. Ability to
prioritize "STAT", "Urgent", and "Routine" requests as necessary
according to release of information policies and procedures.
Complete all processing of STAT requests as a high priority upon
receipt in the absence of any extenuating circumstances. Initiates
call backs to all telephone communications within 24-48 business
hours or immediate callbacks for any STAT/Urgent medical record
needed. Requests paper records, as needed, by utilizing the EHR or
retrieval from offsite storage facility. Communicates with
appropriate staff to coordinate scanning of paper information, as
needed, to complete requests. Coordinates release of information
requests with other Partners facilities as needed. Generate e-fax
action letters to requestors advising of request status or issues
pertinent to a particular request according to release of
information policies and procedures. Answers incoming phone calls
from patients, business associates, clinicians, insurance
companies, etc. Clearly articulates and concisely interprets
hospital and department policies and procedures, HIPAA, and MGL, to
callers and customers of the Release of Information Unit. Utilizes
medical terminology and clinical information to make sound
judgments and to employ problem-solving skills when processing
requests for medical information. Promotes courteous and friendly
interactions with patients, physicians, clinical/support staff,
attorneys, third parties, and other customers, internal and
external. Meets and greets patients and provides immediate point of
service for patients and authorized third parties seeking to
acquire copies of medical records when necessary. Other duties
assigned by the supervisor or manager. Uses the Partners HealthCare

Qualifications High school diploma or equivalent. Associates degree
preferred. Knowledge of medical record and release of information
procedures, at a level generally acquired during two years of
experience is preferred. Understands the components of a medical
record, including how a record is maintained, organized and the
retrieval of medical documentation in both paper and/or electronic
format. General knowledge of medical terminology. General knowledge
of HIPAA privacy and security rules. Knowledge of state and federal
rules and regulations related to the disclosure of protected health
information (PHI) preferred. Experience working with Epic clinical
applications preferred. Skills/Abilities/Competencies Required Self
motivated and able to manage time in a busy office environment with
interruptions. Critical thinking skills and sound reasoning
abilities. Strong customer service and proficient verbal, written
and telephone communication skills. Must be courteous and
cooperative at all times when interacting with auditors and all
levels of departmental and hospital personnel. Must utilize medical
terminology and clinical knowledge to support problem-solving
issues. Detailed oriented skills required. Basic computer skills.
Competent in Adobe, Outlook, Word and Excel tools and other related
computer applications. Prioritizes tasks in a dynamic work
environment with multiple and conflicting demands; successfully
performs different tasks simultaneously. Establishes and maintains
cooperative working relationships with co-workers, outsourced
vendors and customers. Stands, bends, and/or push/pulls record
carts and lift/carry paper medical records. Deliver high quality
customer service utilizing proficient verbal, written and telephone
communication skills. Present as courteous and cooperative at all
times when interacting with peers and all levels of departmental
and hospital personnel. Able to meet deadlines. Must have the
ability to prioritize tasks in a dynamic work environment with
multiple and conflicting demands; successfully performing different
