CHA House Calls Program has a team of doctors and nurse
practitioners who provide primary care to older adults who are
homebound or can't get to the doctor's office. We have been serving
the greater Cambridge and Somerville area for 30 years and are
seeking to expand our coverage in the Metro north area (Malden,
Chelsea, Revere, and Everett). Patients who join the program get
routine follow-up and acute care visits in the most comfortable and
convenient setting - their home. Summary: Under the general
direction of the Manager and in collaboration with a physician with
geriatric medicine expertise, and within established hospital and
departmental policies and procedures, the nurse practitioner (0.75
FTE) co-manages the medical care of a panel of 75 House Call
Program patients. Essential job functions are focused on customer
satisfaction, effective use of resources, teamwork, innovation and

disability. Responsibilities: Performs routine follow-up in-person
home visits every 1-2 months on his/his/their patients, to manage
acute and/or chronic diseases. Therapeutic management of diseases
will adhere to established clinical guidelines. Performs urgent
in-person home visits as necessary to address the patient's medical
needs. Initiates, evaluates and modifies the effectiveness of
therapeutic plans, collaborating appropriately with the partner
geriatrician and other members of the health care team. Orders
laboratory and radiographic studies as necessary for the
determination of health status according to guidelines.
Discriminates between normal and abnormal findings. Triages urgent
telephone calls from his/her/their patients. This may include
telephonic management of issues or scheduling of urgent in-person
home visits based on clinical necessity. Performs care coordination
with involved community agencies, typically through telephone
calls. Performs care management of the patient's global medical,
psychosocial, and functional needs. This includes referrals to
appropriate medical consultants and community agencies (such as
VNAs) and completion of paperwork for DME requests. Communicates
effectively and works as part of a team. Displays ability to get
along with co-workers, other departments and the public. Attends
continuing education conferences and programs to obtain requisite
numbers of CEU's as required by legislation for registration and
certification. Maintains current awareness of new concepts,
methods, techniques, regulations, etc., related to the Nurse
Practitioner role and geriatric medicine. QUALIFICATIONS:
Education: Successful completion of an accredited Nurse
Practitioner program Licensure: Registered to practice in the
Commonwealth of Massachusetts in an expanded role. Maintains DEA
number and Mass DPH registration to prescribe controlled
substances. MA. RN Licensure required Certifications: Must have
taken and passed a recognized national certification exam and have
maintained certification. Current American Heart Association Basic
Life Support C Certification. Must successfully complete The
Cambridge Health Alliance credentialing process. Work Experience: A
minimum of two (2) years nursing experience. Geriatric experience
and/or prior training is strongly preferred. In keeping with
