other providers.The Dialysis Home Therapies Nurse coordinates the
administrative and clinical aspects of the home dialysis program,
including home hemodialysis and peritoneal dialysis treatment
options, and communicates with patients and support staff to ensure
the highest standards of care.Schedule: Fulltime, Monday through
Friday; 32-29 hours/week; on call as neededCompensation: Pay

othersWhat You Can Expect:Collaborate with medical team on patient
care concerns, hospitalizations, discharges and/or modality
changes, etc.Coordinate home training sessions, clinic visits,
patient care conferences, home visits and all clinical and
educational activities to assure efficient and productive use of
nursing hours and program facilitiesMonitor patient supply ordering
and usage while maintaining quality careCommunicate regularly
regarding home program activities with interdisciplinary care
teamMonitor ongoing patient care parametersPerform all registered
nurse functions and dutiesCreate and maintain appropriate record
keeping, patient training and documentation in accordance with ESRD
Network, regulatory agencies, DCI CQI program and the dialysis
facility's policies and proceduresSuccessful Candidates

solveEducation/Training:Accredited nursing degreeDialysis
experience a plusCurrent MARN licenseValid driver's license and
