Job Description:
Element Care seeks a compassionate and dedicated Social Worker
to join our team in the Social Services department, providing
specialized support within our Program of All-Inclusive Care for
the Elderly (PACE). This role is critical in assisting elderly
clients to live a dignified and independent life in the comfort of
their own homes, coordinating healthcare and supportive services
tailored to each individuals needs. Conduct comprehensive
assessments of client's social, emotional, and environmental needs.
Formulate and implement care plans in conjunction with
interdisciplinary teams. Provide counseling and support to clients
and their families on coping with cognitive and aging-related
challenges. Advocate for clients' access to community resources and
services, ensuring their needs are adequately met. Maintain
accurate records and documentation in compliance with regulatory
bodies and funding sources. Participate in team meetings and
contribute to the continuous evaluation and improvement of services
provided. Offer crisis intervention services as needed and
coordinate with other professionals to provide seamless care.
Strong interpersonal and communication skills Ability to work
effectively in an interdisciplinary team Knowledge of community
resources and services for the elderly Crisis intervention skills
Effective time management and organizational skills Empathy and
sensitivity to the needs of the elderly and their families Medical
Insurance Dental Insurance Vision Insurance 401k Licensed Clinical
Social Worker (LCSW) or eligibility to be licensed is required.
Valid driver's license and reliable transportation, as local travel
to client homes may be necessary.
